Vaccine Sweepstakes

The “Your Shot to Win” Sweepstakes is open to all students enrolled in Montana State University in Fall 2021 who complete their official Sweepstakes Vaccine Verification Form. Only one Vaccine Verification Form may be completed per enrolled student. Entrants must be completely vaccinated (i.e., received both Moderna or Pfizer shots or one Johnson & Johnson shot) to be eligible to win. The Sweepstakes is subject to all applicable federal, state, and local laws.

To enter the Sweepstakes, MSU students may complete their online Vaccine Verification Form (or in person at the Office of Student Engagement SUB 221) from July 28, 2021, to December 13, 2021 to be eligible for a Financial Assistance Award and/or Prize.

Sweepstakes facilitators from the Office of Student Engagement (OSE) will randomly select prize winners between August 30, 2021 and December 16, 2021. Prizes will be awarded daily (on weekdays) at approximately noon (12pm). Financial Assistance Awards will be awarded on December 13, 2021. OSE will contact winners via the contact information provided by entrants on their Vaccine Verification Form. Winners of Prizes may select their desired prize from among the pool of prizes available. Any unclaimed prize(s) will be forfeit after December 22, 2021.

During the Entry Period, visit the Sweepstakes webpage at https://www.montana.edu/health/coronavirus/health-safety/vaccinations/win/ and follow the instructions to electronically complete and submit the Vaccine Verification Form. You may also complete the electronic form in person by visiting the Office of Student Engagement (SUB 221). You will be asked to provide basic contact information, including your name, phone number, and email address. You will attest your vaccination by uploading a photo of your vaccination card or by indicating the vaccine you received, date, and location of your vaccination. You must complete all required fields of the entry form with valid information on or before the date of the Sweepstakes drawing to win. By entering to win, you will be asked to voluntarily provide documentation of your vaccination status. Entrants may see the status of their Vaccine Verification Form and entry by visiting their CatsConnect profile. Your entrance to the Sweepstakes is contingent upon Vaccine Verification Form acceptance which will be conducted within 2 business days. Entrants will receive an email verifying their entry into the Sweepstakes.

Entrants may earn additional chances to win by “Referring a Friend.” A referral requires a valid name and last four digits of the referrer’s GID. Entrants may earn multiple sweepstakes entries by referring multiple eligible students. 

All valid entrants will automatically receive a $10 credit on their Cat Card. 

Entrants who are randomly selected as Sweepstakes winners may receive no more than one Prize and one Financial Assistance Award. Any person who attempts or otherwise encourages false entries, as deemed by an OSE facilitator, will be disqualified.

Five (5) Financial Assistance Awards valued at $5,000.00 will be awarded on December 13, 2021 from the Sweepstakes entries. Financial Assistance Awards will be applied toward Spring 2022 educational expenses. By certifying your entry form, you agree to apply the $5,000.00 toward outstanding charges with Montana State University. If no outstanding charges or costs exist (costs covered by other funding resources) the award will be refunded in full or in part based on those outstanding charges. Awards will be processed through the Financial Aid office for the Spring 2022 Term by the 15th class day. Other federal, state or institutional restrictions may apply. LIMIT ONE FINANCIAL ASSISTANCE AWARD PER PERSON.

Seventy-six (76) Prizes, with an average retail value of $500, will be awarded during the Sweepstakes Award Period. Winners may select their desired prize from a pool of available prizes. Any difference between stated approximate retail value and actual value of prizes will not be awarded. There will be no substitution or cash equivalent prizes. Each Prize is awarded “as is” with no warranty or guarantee, either expressed or implied. LIMIT ONE PRIZE PER PERSON.

Winners will be selected in a random drawing from among all eligible entrants collected during the Entry Period taking place on or about noon in the Office of Student Engagement. You do not need to be present to win. The drawing will be conducted by an OSE facilitator, whose decisions and interpretations on all matters relating to the Sweepstakes and these Official Rules are final and binding in all respects. OSE reserves the right to disqualify any entrant or winner and may refuse to award a Prize or Financial Assistance Award to a person who is not eligible, has violated a rule, or obtained winner status using fraudulent means. If there is a dispute about the identity or validity of an entrant’s Vaccine Verification Form, the entrant will be disqualified, and an alternative winner will be awarded. Winners will be notified by telephone and email provided on their Vaccine Verification Form. Any unclaimed prize will be forfeit after December 22, 2021.

The odds of winning will depend on the number of eligible entries received, according to the eligibility requirements in the Sweepstakes.

By entering this Sweepstakes, you promise to abide by the Official Rules and the decisions of the OSE facilitator, which will be final and binding in all respects.
